Job summary

Carease Health seeks a compassionate Mental Health Provider to join its Collaborative Care team. You will deliver emotional support, brief therapy, and mental health assessments to residents in skilled nursing facilities.

This role focuses on direct resident impact, supervision, and collaboration with psychiatric providers to ensure comprehensive care and ongoing professional development.

Qualifications

• Must have a degree in Social Work, Psychology, Counseling, or related mental health field.
• Understanding of geriatric mental health and crisis management for long-term care settings.
• Strong communication, documentation, and relationship-building skills.

Responsibilities

• Visit designated facilities according to schedule with supervisor.
• Assess residents to identify emotional, cognitive, and psychosocial needs.
• Provide short-term, solution-focused therapy and supportive interventions.
• Document visit notes and maintain records of interactions and interventions.
• Collaborate with psychiatric professionals and participate in monthly psychiatry NP meetings.
