Job summary

Psychosocial Therapies seeks a Licensed Social Worker or Licensed Professional Counselor to deliver compassionate mental health services to seniors in nursing homes and assisted living facilities across Ohio. You will assess residents, design individualized care plans, and collaborate with medical teams to ensure holistic care.

Deliver talk therapy using evidence-based methods, monitor progress, and maintain confidential records while supporting families and care staff in challenging situations.

Qualifications

• Must hold a Ohio licensure as an LSW/LCSW or LPC.
• Experience with geriatric populations in long-term care settings preferred.
• Ability to conduct assessments, develop individualized care plans, and monitor progress.

Responsibilities

• Conduct thorough assessments to understand clients’ needs, challenges, and mental health conditions.
• Identify individualized support options based on these evaluations.
• Develop customized care plans for each client, considering their unique circumstances and mental health requirements.
• Work closely with healthcare professionals, nursing staff, and case managers to coordinate services effectively.
• Facilitate seamless communication and holistic care for clients.
• Regularly assess clients’ progress and adjust care plans as needed.
• Document treatments, interventions, and services provided.
• Provide talk therapy using evidence-based techniques to support emotional and behavioral health.
