Qualifications

• Active CNA certification in Florida required.
• Knowledge of Chronic Care Management and Behavioral Health Integration.
• Excellent communication and interpersonal skills.
• Ability to prioritize tasks and manage multiple responsibilities.
• Proficiency in EMR systems and other relevant software.
• Strong computer skills with attention to detail.
• Compassionate, patient-centered approach.

Responsibilities

• Provide care in skilled nursing and assisted living facilities daily.
• Develop and implement patient care plans using electronic systems.
• Communicate daily with patients, POAs, facility staff, and families.
• Conduct medication reviews and discuss mental health diagnoses.
• Utilize psychiatric scales for assessments.
• Schedule visits for patients needing additional support.
• Monitor adherence to care plans and evaluate effectiveness.
• Document patient progress timely.
• Support Behavioral Health Integration initiatives.
• Visit patients independently in nursing homes and complete charting in office.
