Balanced Wellbeing LLC is seeking a dedicated Mental Health CNA to support geriatric patients in skilled nursing and assisted living facilities. You will conduct assessments, update care plans, coordinate with families, and participate in Behavioral Health Integration efforts.

This role offers a Monday–Friday schedule 8:30 AM–5:00 PM with no weekend shifts and a compensation package including hourly pay of 17–19 USD and potential bonuses up to 5,000 USD per month, reflecting the value of

Qualifications

Excellent communication and interpersonal skills.
Proficiency with electronic medical record (EMR) systems.
Ability to prioritize tasks and manage multiple responsibilities.

Responsabilités

Provide care in skilled nursing and assisted living facilities on a daily basis.
Develop and implement patient care plans using electronic systems, including goal setting and progress monitoring.
Communicate daily with patients, POAs, facility staff, and families.
Conduct medication reviews with patients and assess mental health status.
Utilize psychiatric scales for assessments.
Schedule visits for patients needing additional support.
Monitor adherence to care plans and evaluate effectiveness.
Document patient progress in a timely manner.
Support Behavioral Health Integration (BHI) initiatives.
Visit patients independently in nursing homes and complete charting in the office setting.
