Responsibilities

• Provide care in skilled nursing and assisted living facilities.
• Develop and implement patient care plans using EMR systems.
• Communicate with patients, POAs, facility staff and families.
• Conduct medication reviews and discuss mental health diagnoses.
• Utilize psychiatric scales for assessments.
• Schedule visits and monitor adherence to care plans.
• Document patient progress promptly and accurately.
