Job summary

Eventus WholeHealth in Indiana is seeking a licensed clinical social worker to provide recovery-oriented rehabilitation services in group or individual settings for residents in skilled nursing and assisted living facilities.

You will collaborate with a multidisciplinary team, develop and implement treatment plans, maintain documentation, and ensure compliance with HIPAA and regulatory standards. A master’s degree in social work and an active state license are required.

Qualifications

• PhD or doctoral-level psychologist license required.
• Master’s degree in social work and state license to practice required.

Responsibilities

• Assess each resident and develop treatment plans to meet needs.
• Consult with primary care physicians, families, and facility staff.
• Document treatment plans and sessions according to procedures.
• Review facility care needs with staff on arrival and departure.
• Maintain open communication with facility staff and administration.
• Ensure compliance with HIPAA, policies, and regulatory guidelines.
