Job summary

Carease Health in Fort Bend County, TX is seeking a compassionate Care Companion/Social Worker to provide emotional support, brief therapeutic interventions, and mental health assessments for residents in skilled nursing facilities.

You will visit facilities, assess needs, and deliver short-term therapy while documenting notes and coordinating with psychiatric professionals in a collaborative care setting. This role emphasizes resident well-being and ongoing training opportunities.

Qualifications

• Associates, Bachelor's or Master's in Social Work, Psychology, Counseling or related field.
• Geriatric mental health knowledge and long-term care crisis management.
• Strong communication, documentation and relationship-building skills.

Responsibilities

• Visit designated facilities in accordance with the schedule set with your supervisor.
• Conduct assessments of residents to identify emotional, cognitive, and psychosocial needs.
• Implement short-term, solution-focused therapy and supportive interventions to address immediate concerns.
• Document visit notes and maintain detailed records of interactions and interventions after each visit.
• Collaborate with psychiatric professionals to ensure comprehensive care, including participation in a monthly meeting with the psychiatric NP.
