The Permanente Medical Group (TPMG) in Napa, CA seeks a Geriatrician/Skilled Nursing Physician for a full-time role covering four facilities and home-based palliative care. You will care for skilled and long-term patients, with focus on improving quality of life for vulnerable populations.

Geriatrics or Hospital Medicine training is preferred. Salary ranges from $320,040 to $330,000 annually, with incentives up to $44,350.

Qualifications

• Must be a licensed physician with geriatric or hospital medicine training.
• Experience in skilled nursing facilities and long-term care preferred.
• Interest in improving quality of life for vulnerable populations.

Responsibilities

• Cover skilled nursing facility patients across Napa locations.
• Follow long-term care patients in outpatient/home-based settings.
• Participate in care coordination and multidisciplinary rounds.
