Job summary

TEEMA is seeking a Licensed Vocational Nurse (LVN) in Stockton, CA to join our clinical operations team. You will work with Providers and the Clinical Nurse Manager to deliver high‑quality care for seniors in a clinic setting, including wound care, POC testing, and medication administration.

The role emphasizes direct patient care, care coordination, and collaboration with an interdisciplinary medical team to support aging participants and their families.

Qualifications

• Graduate of an accredited School of Nursing (Associate’s Degree or community college equivalent).
• Active, unencumbered Licensed Vocational Nurse (LVN) license in California.
• Minimum of one year nursing experience with frail/elderly population.
• Deep clinical knowledge to treat frail, elderly individuals and manage complex medical situations.
• Reliable transportation to travel to the clinic location.

Responsibilities

• Direct Patient Treatment: perform wound care and clinical procedures as directed by PCP; document care.
• Medication Administration: safely administer meds in clinic and reinforce patient education.
• Medication Reconciliation: review and reconcile medications during visits; monthly inventory of clinical med systems.
• Specialty Orders: process specialty pharmacy orders and address patient inquiries.
• Transition Coordination: manage same-day med order changes from outpatient procedures and appointments.
• High‑Risk Monitoring: conduct high-risk touchpoints and document vitals and reminders.
• Proactive Follow-ups: perform follow-ups for Yellow/Green risk participants as per RN Case Manager guidelines.
• Perioperative Support: complete pre- and post-operative outreach and follow-up care.
