Job summary

Deer Oaks Behavioral Health is seeking a Licensed Professional Counselor (LPC) to join an integrated clinical team in long‑term care facilities. The role offers a Monday–Friday schedule, onsite work, and strong administrative and clinical support to focus on patient outcomes.

You will deliver individual, family, and group therapy, perform assessments, and participate in care planning and interdisciplinary collaboration to support geriatric residents with complex medical and behavioral needs.

Qualifications

• Active, unrestricted LCPC license.
• Valid driver’s license and reliable transportation.
• Strong written, verbal, and interpersonal communication skills.
• Ability to work independently while collaborating with an interdisciplinary team.
• Comfortable navigating LTC environments.

Responsibilities

• Provide individual, family, and group therapy to LTC residents.
• Conduct clinical assessments, psychosocial evaluations, screenings, and treatment planning.
• Collaborate with nursing, social services, rehabilitation, and medical providers to coordinate care.
