Job summary
Vital HealthCare Solutions in the Northeast region is seeking Licensed Clinical Social Workers (LCSWs) and Licensed Professional Counselors (LPCs) for full-time, flexible schedules. The role focuses on geriatric mental health within skilled nursing and assisted living facilities, delivering evidence-based individual psychotherapy and comprehensive assessments.
You will collaborate with psychology and psychiatry teams, develop tailored treatment plans, and connect with diverse patient populations
Qualifications
- Active LCSW or LPC license (required)
- Experience in geriatric mental health, long-term care, or behavioral health (preferred)
- Compassionate, collaborative, and motivated clinicians in interdisciplinary settings
Responsibilities
- Deliver individual psychotherapy and counseling using evidence-based practices
- Conduct comprehensive psychological evaluations and diagnostic assessments
- Develop and implement tailored treatment plans based on patient needs
- Collaborate with Psychology and Psychiatry teams and facility staff for integrated care
- Work with a diverse population with a wide range of emotional and behavioral health needs
