Job summary

Community LIFE in Bedford, PA, is seeking a Home Care Coordinator to collaborate with the interdisciplinary team to assess participant needs and coordinate home and personal care services. The goal is to maximize rehab potential and minimize inpatient care while supporting smooth transitions home after hospital stays.

The role requires a PA COTA license, PA driver’s license, and at least one year of experience with frail elderly populations. Weekend/on-call rotations are included in the schedule.

Qualifications

• Graduate of an accredited COTA program.
• At least 1 year of experience with frail elderly population.
• Valid PA COTA license and PA driver’s license.
• Knowledge of geriatric needs and community health.
• Strong written and oral communication; ability to manage priorities.
• Ability to work with interdisciplinary team.
• Working knowledge of Windows, email, and word processing.

Responsibilities

• Assess participant needs and coordinate home/personal care services.
• Maximize rehab potential in daily living.
• Support transitions from hospital to home after hospitalization or rehab.
• Collaborate with therapy team to formulate care plans.
• Participate in weekend and on-call rotations.
