Job summary

CarePartners Health Services in Asheville, North Carolina, part of HCA Healthcare, seeks a Registered Dietitian for the PACE program to assess nutritional status, develop care plans, and provide counseling within the interdisciplinary team.

The role coordinates meals with kitchen and contracted providers, educates caregivers, and supports nutrition needs across the care continuum for elderly participants.

Qualifications

• Bachelor degree in food and nutrition or dietetics.
• MS degree preferred with emphasis in nutrition.
• Licensed Dietitian/Nutritionist in NC and CDR registration.
• Minimum 1 year with frail/elderly population; 2 years clinical nutrition and supervisory experience.
• RD/RDN must be obtained within 2 years of employment start date.

Responsibilities

• Assess nutritional status of PACE participants and plan interventions.
• Provide nutrition counseling, education and therapy to participants.
• Coordinate Center meal service and with contracted meal services.
• Educate and counsel family and other caregivers regarding dietary issues.
