Job summary
The Pavilion Rehabilitation and Nursing Center in Hyannis, MA is seeking a full-time Registered Dietitian to join our team and support medical nutrition assessment, care planning, and ongoing resident evaluation. You will work with physicians and nurses to optimize nutrition care across short-term and long-term residents.
Qualifications include a Registered Dietitian license and 3 years of geriatric or clinical nutrition experience.
Qualifications
- Registered Dietitian License and active membership in the American Dietetic Association.
- Three years’ experience in geriatric, administrative, or clinical nutrition.
- Proficient computer skills.
- Ability to perform duties with or without reasonable accommodations.
Responsibilities
- Perform medical nutrition assessments and develop nutrition and hydration care plans.
- Conduct ongoing evaluation of all residents and monitor nutrition status.
- Collaborate with physicians and nursing staff to develop care plans.
- Monitor weight data trends and calorie counts; report concerns.
- Ensure compliance with MDS/CAAs and documentation.
- Provide patient and family education on nutrition interventions.
