Job summary

Chambersburg Skilled Nursing & Rehab is seeking a qualified Dietitian to lead nutrition care for residents. You will work with an interdisciplinary team to develop and implement nutrition plans, ensure documentation, and monitor outcomes. Prior health care or consulting dietitian experience and geriatric knowledge are preferred.

Qualifications

Dietitian registered and/or licensed as required by the state of employment.
Experience in a health care setting or as a consulting dietitian preferred.
Experience with geriatric population is preferred.

Responsibilities

Advise leadership on nutrition care and regulatory standards.
Collaborate with interdisciplinary team to design nutrition care plans for residents.
Track and evaluate effectiveness of nutritional interventions.
Document nutrition assessments, interventions, and follow-up clearly.
Address nutrition concerns with team members and families as needed.
Provide diet instruction and education for residents, families, and caregivers.
Monitor the nutrition care delivery process to ensure clinical excellence.
