Responsibilities

• Assess psycho-social needs of assigned patients and initiate discharge planning.
• Counsel patients and families regarding care after discharge.
• Coordinate referrals to home health and community agencies as needed.
• Maintain knowledge of relevant mental health laws and regulations.
• Collaborate with the multidisciplinary team for treatment planning.
• Conduct crisis intervention as indicated.
• Conduct individual and group therapy sessions as required.
• Support program planning, development and evaluation.
• Ensure documentation and communication with physicians and care teams.
