The Sisters of Mercy of the Americas are seeking an experienced RN Case Manager to support Sisters in residence and the surrounding community. This role blends nursing, case management, advocacy, education, and care coordination to ensure compassionate, coordinated care aligned with individual goals.

Ideal candidates will have an RN license (BSN preferred) and 5+ years in geriatric or related care settings, with strong organizational, communication, and collaborative skills for a faith-based

Qualifications

• Current Registered Nurse license; BSN preferred.
• 5+ years of professional nursing experience.
• Geriatric, long-term care, homecare, or related setting experience.
• Experience with case management and discharge planning.
• Independent worker and team collaborator.
• Strong organizational skills and attention to detail.
• Calm and effective response to emergencies.
• Strong communication and interpersonal skills.
• Holistic health and wellness approach.
• Comfort serving religious women in healthcare settings.

Responsabilités

• Conduct wellness and health assessments and identify changing health needs.
• Provide case management for chronic conditions and new diagnoses.
• Develop and implement individualized care plans and participate in discharge planning.
• Coordinate care with medical providers, specialists, and community resources.
• Accompany Sisters to medical appointments and navigate healthcare system.
• Coordinate follow-up appointments, transportation, and communications.
• Provide hands-on nursing care: vitals, wound care, injections, first aid.
• Respond to health-related emergencies and crises.
• Educate and support on health topics with the care team.
