Job summary

Abode Care Partners seeks a Director of Behavioral Health Practice to lead the geriatric behavioral health program in skilled nursing facilities and related settings. The role emphasizes psychiatric medication management, therapy services, and cognitive screening within a value-based care framework.

The ideal candidate is a PMHNP with extensive experience in elder care, capable of leading multi-provider teams across Kentucky and Tennessee, while driving quality outcomes and cost-effective care

Qualifications

• Master’s degree or higher in nursing.
• Current PMHNP certification required.
• Active, unrestricted APRN license.
• Kentucky and Tennessee APRN licenses preferred; willingness to obtain additional licenses.
• Three or more years in behavioral health clinical care.
• Experience in geriatric psychiatry, long-term care, or facility-based behavioral health preferred.
• DEA registration as applicable to scope of practice.
• Must travel within markets (KY/TN) as needed.

Responsibilities

• Provide high‑quality behavioral health care for older adults in SNFs and related settings, including evaluation and medication management.
• Lead daily clinical operations of the behavioral health practice, ensuring coverage, workflows, and access.
• Oversee, mentor, and develop clinicians and therapists across multiple facilities.
• Partner with facility leadership and interdisciplinary teams to deliver coordinated care.
• Support screening, diagnosis, and management of common geriatric behavioral health conditions.
