Job summary

Medical Home Network seeks a Geriatric Advanced Practice Provider to deliver high-quality, patient-centered care for older adults within MHN’s network of FQHC partners in Illinois. You will lead preventive care, care coordination, AWVs, and ACP discussions, while shaping clinical programs for aging populations.

You will collaborate with physicians, nurses, social workers, and care managers to improve outcomes, scale geriatric initiatives, and sustain high-quality care across Springfield and

Qualifications

• Active NP license (IL)
• National NP certification (AANP or ANCC) or PA certification
• Geriatric care experience
• Experience conducting AWVs and ACP discussions
• Strong communication skills
• Experience in FQHCs or similar community health settings preferred
• Experience building or scaling clinical programs
• Experience in primary care or population health
• Background in value-based care, risk adjustment, or quality initiatives
• Familiarity with EHR documentation and Medicare compliance
• Experience working with interdisciplinary or care management teams
• Spanish language skills a plus

Responsibilities

• Provide comprehensive primary and preventive care for MHN-enrolled aging patients.
• Perform AWVs and ACP discussions with patients.
• Develop patient-centered care plans with patients, caregivers, and teams.
