About the Role

As a Primary Care Physician, you will serve as the cornerstone of care for an established panel of adult and geriatric patients. Working alongside a dedicated interdisciplinary care team, you will focus on improving health outcomes, preventing hospitalizations, and helping patients successfully manage chronic conditions.

Common conditions managed include:

• Diabetes
• Hypertension
• Congestive heart failure
• Coronary artery disease
• COPD and respiratory disorders
• Chronic kidney disease
• Arthritis and mobility limitations
• Multiple chronic comorbidities common within the Medicare population

This role is ideal for a physician who values continuity of care, patient education, and long-term relationships.

Compensation & Benefits

Compensation Range: $192,153.00 to $288,230.00. The anticipated base salary range represents the Company's good-faith estimate of the compensation it reasonably expects to pay for this position at the time of posting. Actual compensation will be determined based on factors including experience, skills, qualifications, geographic location, internal equity, and business needs.
