CSR Required

Dates needed: Nov. 29, Dec 2-6, 9-12
Hours: Mon-Fri 8a-5p

Clinical Staff : 1 associate 2 days a week. 2 Hygienists and 3 DAs
Require Procedures : Surgical extractions, implant crown placement
Avg PPD: 15-25
Additional Requirements : None
Reason for need: Vacation Coverage
