You will provide in-person and telehealth care, diagnose and manage conditions, and develop personalized treatment plans. Strong communication and teamwork are essential as you collaborate with a multidisciplinary team.

Qualifications

• Active Oregon NP license or ability to obtain
• Board Certified: FNP, AGNP, or AGNP-PC
• 2+ years of clinical experience preferred
• Interest in functional, preventive, or lifestyle medicine
• Strong communication and teamwork skills

Responsibilities

• Provide in-person and telehealth care
• Diagnose and manage acute and chronic conditions
• Develop personalized treatment plans (lifestyle, supplements, hormone therapy, medications)
• Educate patients on preventive and functional medicine
• Collaborate with a multidisciplinary care team
• Document in NextGen EHR
