Job summary

Alto Evergreen Walk Assisted Living in South Windsor, CT is seeking a compassionate Wellness Nurse (LPN) to lead a care team, deliver high-quality resident care, and support daily operations on 2nd shift and weekends.

You’ll supervise caregivers, administer meds, respond to changes in condition, and uphold policy and best practices. LPN license in good standing and CPR/AED required; minimum 1 year senior living experience.

Qualifications

• 18 years or older.
• Active LPN license in good standing with the state.
• CPR/AED certification.
• At least 1 year of LPN experience in a senior care setting.
• Positive criminal background screening.
• Commitment to a drug-free workplace.
• Experience with computerized resident documentation systems is a plus.

Responsibilities

• Supervise and support our care team while monitoring resident wellbeing in Assisted Living.
• Guide daily assignments, coach associates, and help resolve problems as they arise.
• Provide exceptional resident care in accordance with provider orders, nursing best practices, and company policies.
• Accurately administer prescribed medications and monitor changes in resident condition.
• Respond to changes in condition, medications, or special care needs.
• Provide first aid and other nursing support as required.
• Perform other duties as assigned.
