Job summary

Hospice of the Piedmont, Inc. is seeking a full-time CNA to provide personal care and home management at patients' residences across Asheboro and High Point service areas, with mileage reimbursement.

Schedule includes Saturdays and Sundays 8am-5pm plus three additional weekdays to be set at hire; weekend differential applies. The role supports patient comfort under RN supervision, documents care, and collaborates with the interdisciplinary group to meet goals, while maintaining patient dignity

Qualifications

• High School diploma or better.
• 1 year experience as a nurse aide (CNA).
• CNA certification/licensure required.
• Valid driver’s license.

Responsibilities

• Follows the nurse aide care plan to provide compassionate care to the patient in the areas of personal hygiene, elimination, feeding, and mobility.
• Reports changes in patient condition to nursing staff and participates in interdisciplinary team conferences as necessary.
• Works with the IDG and departments to achieve patient care goals.
• Documents patient care clearly, completely, concisely, and timely.
• Manages time, supplies, and technology; maintains a safe, clean patient environment.
• Schedule visits in coordination with patients, families, and facility staff.
• Teaches home safety, energy conservation and sanitation; provides light housekeeping.
• Assists with meal preparation and related tasks.
• Demonstrates strong communication and interpersonal skills with patients and families.
• Follows nurse aide standards; participates in staff training.

Job description

This position is a full-time, hourly, non-exempt position
Work Schedule: every Saturday and Sunday 8am-5pm required plus 3 additional weekdays to be set at the time of hire, weekend shift differential applies.
Location: travels to homes and facilities between both of our Asheboro and High Point service areas (serving full 8 county territory), will provide mileage reimbursement.

This position provides personal care and home management tasks at patient’s residence, under the supervision of a RN Case Manager.

What are the essential duties day to day?

• Follows the nurse aide care plan to provide compassionate care to the patient in the areas of personal hygiene, elimination, feeding, and mobility.
• Reports changes in patient’s condition, concerns about patient and/or family to appropriate nursing staff in a timely fashion and participates in interdisciplinary team conferences as necessary.
• Works collaboratively with the IDG and other departments in achieving patent care goals.
• Documents patient care according to agency policy and procedures; documentation is clear, complete, concise, and timely.
• Responsible for professionally managing time, people, medical supplies, and technology.
• Establishes and adheres to a timeframe for patient visits in coordination with patients, families and facility staff members.
• Maintains a clean, safe and comfortable environment for the patient.
• Teaches and/or reinforces home safety, energy conservation and sanitation.
• Provides light housekeeping tasks.
• Assists with, or provides, meal preparation and related tasks.
• Demonstrates excellent communication skills and creates successful interpersonal relationships with patients and families.
• Complies with organizational nurse aide standards.
• Participates in staff training and education programs.
• Average of 6-7 visits a day
