Job summary

Qualifications

• Valid NYS License & CCC-SLP.
• Experience with cognitive and dysphagia rehab in Skilled Nursing Facility.
• Knowledge of Rehab Optima preferred.

Responsibilities

• Provide quality SLP services, including evaluating patients, screening, initiating referrals, obtaining data and following through with plan of care.
• Completes timely documentation including progress notes, screen reports, daily notes, and discharge summaries.
• Knowledgeable in short term and long term SLP care.
• Instruct patients as well as family members and staff in goals, progress, and appropriate therapeutic techniques.
• Ensures compliance with protocols and regulations.
