Job summary

Concept Rehab in Port Huron, MI is seeking a Full-Time Speech-Language Pathologist (SLP) to join a therapist-owned organization dedicated to excellence in patient care.

You will evaluate patients, create individualized treatment plans, and provide speech, language, cognitive, and swallowing therapy in a Skilled Nursing Facility, collaborating with nursing and interdisciplinary teams while documenting progress and educating families.

Qualifications

• Must hold current Michigan SLP license.
• ASHA CCC-SLP as preferred credential.
• Master's degree in Speech-Language Pathology.
• Experience in SNF, rehabilitation or geriatric care preferred.

Responsibilities

• Evaluate patients and develop individualized treatment plans.
• Provide speech, language, cognitive and swallowing therapy.
• Monitor patient progress and adjust treatment plans as needed.
• Collaborate with nursing, therapy and interdisciplinary teams.
• Complete accurate and timely clinical documentation.
• Educate patients, families and caregivers.
