Job summary

Continuum Therapy Partners, LLC seeks a Speech-Language Pathologist to evaluate and treat speech, language, and swallowing disorders. The clinician will develop individualized plans, monitor progress, and educate patients and families on communication strategies.

Full-time daytime schedule at Heritage Manor in Chisholm, MN, with a comprehensive benefits package and licensure reimbursement. Minimum state licensure required; CCC-SLP preferred with 1 year of experience.

Qualifications

• Master's Degree in Speech-Language Pathology is required.
• Current, unencumbered state SLP License; ASHA CCC-SLP is required.
• 1 year of previous experience as a Speech-Language Pathologist preferred.

Responsibilities

• Evaluates and develops individualized treatment plans for speech, language, and swallowing disorders
• Documents patient progress and adjusts treatment plans based on clinical outcomes
• Educates patients and families on communication strategies and swallowing safety
