Job summary

Dover Behavioral Health System in Dover, DE is seeking a dynamic Social Worker to provide quality case management and psychotherapeutic services to acute adolescent and adult patients in a full-time role. You will conduct psychosocial assessments, develop treatment plans, deliver individual, group, and family therapies, and coordinate post-discharge care with the treatment team.

The position requires a Master’s degree in Social Work and 1–3 years’ inpatient behavioral health experience;

  • Master's degree in Social work or a related field.
  • 1–3 years of experience in a related field, preferably inpatient behavioral health.

Responsibilities

  • In an inpatient hospital setting conducts psychosocial assessments to document patient history and identify issues for treatment.
  • Deliver high quality clinical services to acute child, adolescent and adult patients.
  • Develop and implement treatment plans with concise, realistic goals.
  • Provide individual, group and family therapies using various modalities.
  • Document treatment in the medical record in a timely manner.
  • Coordinate discharge planning and identify community resources for aftercare.
  • Participate in treatment team meetings and present case progress to the interdisciplinary team.
  • Serve as the patient’s and family’s primary contact and advocate during stay.
  • Report incidents to appropriate authorities as required.

Our treatment center provides mental health and addiction treatment services to individuals from Delaware, Maryland, and most other states throughout the US. Dover Behavioral Health Systemis seeking a dynamic and talented Social Worker. The incumbent will be responsible for providing quality case management and psychotherapeutic services to acute adolescent and adult patients. This position is a full-time (40 hours per week) salaried position.

Key Responsibilities include:
  • In an inpatient hospital setting conducts psychosocial assessments to document patient history and identify issues for treatment by interviewing the patient and their family members.
  • Deliver high quality clinical services to acute child, adolescent and adult patients
  • Participate in the development and implementation of treatment plans, ensuring that concise, realistic care plans are carried out.
  • Provide individual, group and family process-oriented therapies as well as conduct other didactic groups using various professional treatment modalities.
  • Document treatment provided to patient in medical record legibly and in a timely manner.
  • Develop and coordinate an individualized discharge plan for the patient by utilizing the treatment team and chart information to determine aftercare needs. Identify and assess family or community resources such as the facility’s outpatient programs, group homes or mental health practitioners to meet the patient’s aftercare needs.
  • Conduct and attend regular treatment team meetings to provide social work perspective to total case management of the patient with the interdisciplinary team by presenting the patient’s case and progress towards treatment plan goals, making treatment recommendations, and actively coordinating each case with all members of the treatment team.
  • Act as the patient’s and family's primary contact during the patient's stay at the facility, as well as their advocate with the treatment team.
  • Report any incidents or suspected incidences of sexual/physical abuse or other issues to community/state agencies, and represent the facility in any legal proceedings.
  • Conducts psychosocial assessments and discharge planning on an on-call, rotating weekend schedule
