Job summary

Relient Health is seeking a full-time Speech Language Pathologist (SLP) for a Skilled Nursing Facility in Rock Top, TN. You will deliver comprehensive assessment and treatment for communication and swallowing disorders, collaborating with a multidisciplinary team to promote patient independence and quality of life.

Qualifications include graduation from an accredited SLP program and a TN state license (eligibility ok). CCC-SLP preferred; supervision available for CFs.

Qualifications

• Graduate of an accredited Speech-Language Pathology program.
• Current TN SLP license (or eligibility).
• CCC-SLP preferred, CFs will be considered when supervision is available.
• Strong interpersonal and documentation skills.

Responsibilities

• Evaluate and treat patients with speech, language, swallowing, and cognitive-communication disorders.
• Develop and implement individualized treatment plans.
• Document patient progress using EMR systems.
• Collaborate with PTs, OTs, nurses, and physicians to deliver coordinated care.
• Educate patients, families, and staff on treatment goals and progress.
