Job summary

Manchester Center for Rehabilitation and Healing in Tennessee is seeking a Speech Language Pathologist (SLP) to join a Skilled Nursing facility team. The role requires TN licensure and the ability to evaluate and treat independently, with a focus on geriatric care and functional therapy activities.

The SLP will assess patients, develop and monitor treatment plans with specific goals, document progress, and educate patients and families.

Qualifications

• Must have graduated from an accredited Speech Therapist college or university.
• Must hold a valid professional license and TN registration.
• Experience with long-term care or geriatric care preferred.
• Strong documentation and record-keeping skills.
• Ability to evaluate patients and document progress independently.
• Collaborates with all disciplines to provide patient-centered care.
• Adheres to confidentiality, HIPAA, and Residents' Rights.

Responsibilities

• Assess referred patients for rehabilitation needs and plan care.
• Establish and administer treatment programs with specific goals.
• Document patient status, progress notes, and discharge documentation.
• Educate patients and families using appropriate therapy methods.
