Job summary

Westlake Health & Rehabilitation in Greeley, CO is seeking a full-time Speech Language Pathologist for our in-house rehab program. The SLP will screen and treat patients with communication and swallowing disorders in a post-acute setting, adhering to regulatory and clinical standards. A valid SLP license is required.

Responsibilities

• Screen and evaluate patients with communication or swallowing disorders and develop treatment plans.
• Provide rehabilitative, skilled therapies per regulatory and clinical standards.
• Deliver therapy interventions including therapeutic exercises and modalities as part of the rehabilitation plan.
• Offer counseling to patients and families regarding communication and swallowing disorders.
• Prepare discharge summaries compliant with regulations and clinical requirements.
