Job summary

Park Place of Elmhurst seeks a Full-Time Registered Nurse (RN) for Campus Care to support delivering quality care to residents within a not-for-profit five-star CCRC community in Elmhurst, IL. The RN will coordinate campus-wide home health, hospice, and private duty services, serving as the primary nurse for assigned cases.

The role emphasizes adherence to regulations, teaching and supervising staff, and developing comprehensive care plans in collaboration with physicians and families.

Qualifications

• Graduated from a state accredited nursing program.
• Current license to practice professional nursing in the state of practice.
• Minimum 3 months experience in public or home health nursing.
• Minimum 1 year of medical/surgical experience.
• Bachelor’s Degree in Nursing with 6 months experience in nursing.

Responsibilities

• Provide campus-wide home health, hospice, and private duty services.
• Serve as primary care nurse for all assigned cases and provide skilled nursing services.
• Teach and supervise clients, caregivers, and home health staff.
• Develop a client plan of care using physician orders.
• Communicate goals of care to client and caregiver.
• Complete medical record documentation.
• Coordinate follow-up and notify physicians of lab results or status.
• Participate in care conferences and on-call system.
