Frontpoint Health is seeking a Home Health Registered Nurse (RN) to provide skilled in-home nursing care. The role involves assessing patient needs, creating and adjusting care plans, and educating patients and families on health practices and disease prevention.

The RN will administer treatments, monitor progress, and collaborate with the care team to ensure high-quality outcomes, while meeting state licensing requirements and following professional standards.

Qualifications

• Current RN license in Texas and CPR certification are required.
• Minimum of 1 year of nursing experience.
• Home health or hospice experience preferred.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a team.

Job Summary

The Home Health - Registered Nurse will provide skilled nursing care to patients in their homes. They will assess patient needs, develop care plans, and implement and evaluate nursing care. The Home Health - Registered Nurse will also educate patients and their families on proper healthcare practices and disease prevention.
