As a Registered Nurse at Unity Health and Rehabilitation Center, you will be responsible for:

• Providing high-quality patient care, including assessing, planning, implementing, and evaluating individualized care plans


• Collaborating with interdisciplinary teams to develop and implement comprehensive care plans


• Conducting regular patient assessments and reporting findings to the Medical Director and other healthcare professionals


• Developing and implementing individualized education plans for patients and their families


• Participating in quality assessment and improvement activities to ensure optimal patient outcomes
