Job summary

Aroostook Mental Health Services, Inc. seeks a compassionate Adult Community Case Manager in Machias to support adults with long-term mental health challenges in their communities.

You will work with a multidisciplinary team to identify goals, link clients to essential resources, and promote independence and wellness. Responsibilities include building recovery-oriented relationships, coordinating services, and providing supportive counseling.

Qualifications

• Associate's Degree or equivalent credits in a Human Services field is required.
• Bachelor’s degree is preferred.
• Must be MHRT-C certified at full or provisional level.
• Vehicle and valid driver’s license required for travel.

Responsibilities

• Build therapeutic relationships using a recovery-oriented, person-centered approach.
• Coordinate individualized services based on each client's plan.
• Connect clients with housing, medical, behavioral health, and community resources.
• Provide supportive counseling and psychoeducation to clients and families.
• Maintain accurate case records and meet documentation requirements.
