Grace Health & Rehab Center in Slaughter, LA, is seeking a full-time Physical Therapist Assistant to join our therapy team. You will provide treatment under supervision, implement individualized plans, and help residents improve mobility and independence in a skilled nursing setting.

The ideal candidate is a licensed PTA in Louisiana, with CPR certification, and strong communication and teamwork skills. On-site, full-time position with benefits and professional development opportunities.

Qualifications

• Current Louisiana Physical Therapist Assistant license.
• Graduate of an accredited Physical Therapist Assistant program.
• Current CPR certification or ability to obtain.
• Previous skilled nursing or long-term care experience preferred.
• Strong communication, organizational, and interpersonal skills.
• Ability to work effectively as part of a team.
• Compassionate and resident-focused approach to patient care.

Responsabilités

• Provide physical therapy treatment under the direction and supervision of a licensed Physical Therapist.
• Implement individualized treatment plans based on each resident’s needs and goals.
• Assist residents with therapeutic exercises, functional mobility, transfers, balance, gait training, and other skilled interventions.
• Monitor and document residents’ progress and response to treatment.
• Communicate effectively with the Physical Therapist, nursing staff, residents, families, and other members of the care team.
• Promote resident safety and encourage maximum independence with daily activities.
• Maintain accurate and timely clinical documentation.
• Follow all facility, state, and professional standards of practice.
