Responsibilities

• Conduct screening and evaluation of patients with physical functioning disorders; develop care plans.
• Deliver skilled, medically necessary treatment interventions.
• Assist with patient transfer, ambulation, and wound care as part of rehab.
• Administer procedures as part of the rehabilitation plan and provide patient-family counseling.
• Generate discharge summaries compliant with regulatory and clinical requirements.
