Halcyon Home is hiring a Home Health LVN (Full Time or PRN) to support geriatric patients in San Marcos, TX and surrounding areas. Competitive pay and supportive team environment are offered.

Responsibilities include under RN supervision patient needs assessment, documentation, care plan execution, and coordination with caregivers and healthcare professionals. Homecare Homebase experience is a plus.

Qualifications

• Graduate of an accredited school of vocational/practical nursing.
• Active state license as LVN/LPN; multi-state license is a plus.
• Minimum 1 year of clinical LVN/LPN experience; home health preferred.

Responsabilités

• Under RN supervision, identify patient needs across physical, psychosocial, and environmental domains with documentation.
• Be present in partnering communities and client residences.
• Communicate effectively with patient, family, team, and other health professionals; complete timely documentation.
• Monitor cases for third-party payor requirements and compliance.
• Participate in agency trainings and QAPI activities.
• Collaborate with patient, caregiver, and RN to implement the care plan and provide ordered services.
• Apply infection control measures and provide patient education.

Essential Functions:

• Under the direction of the RN, assists in identifying the patient’s physical, psychosocial, and environmental needs as evidenced by documentation.
• You will be expected to be present within our partnering communities and our clients independent residences.
• Provides effective communication to the patient, family, team members, and other healthcare professionals as evidenced by clinical notes, case conferences, and communication notes. Completes appropriate documentation in a timely manner.
• Monitors assigned cases to ensure compliance with requirements of third-party payor.
• Participates in the Agency sponsored in-service trainings.
• Promotes the Agency philosophy and administrative policies to ensure quality of care.
• Participates in planning and implementing care in partnership with the patient, representative (if any), caregiver(s), and the RN. Provides services that are ordered by the physician as indicated in the plan of care.
• Provides care using infection control measures that protect both staff and patient.
• Provides patient and caregiver education.
• Participates in the Agency’s QAPI program.
• Demonstrates commitment, professional growth, and competency.
• Assures the continuity of care through delivery of quality patient care.

Requirements

Education:

Graduate of an accredited school of vocational/practical nursing.

Licensure:

Active, state license as a licensed vocational/practical nurse, or a multi-state license issued by a Nurse Licensure

Experience:

One year of experience as a licensed vocational/practical nurse in a clinical care setting required.

• Home health experience, preferred
• Homecare Homebase platform experience is a PLUS!

Skills:

Nursing skills as defined as generally accepted standards of practice. Good interpersonal skills. Proof of current CPR certification.

Transportation:

Reliable transportation and valid auto liability insurance.

Payrate:

$35-$40 per visit
