Job summary

We Care Health Centers seeks a Nurse Practitioner to provide primary and preventive care for residents in long-term care facilities. The clinician will manage acute changes, perform advanced care planning, and coordinate with an interdisciplinary team.

Applicants must hold an OH NP license, board certification, and DEA licensure. Experience in long-term care and EMR proficiency are required; psych experience is a plus.

Qualifications

• Current, unrestricted NP license in OH.
• Board Certification.
• DEA licensure.
• Clinical background in adult, family or geriatric specialties.
• Experience in long-term care setting.
• Ability to work with an EMR.
• Ability to manage patients with advanced illness and multiple chronic conditions.
• Ability to work in an interdisciplinary care team.
• Psych experience a plus.

Responsibilities

• Provide primary and preventative care for long-term care patients in SNFs.
• Manage residents with acute changes in condition.
• Advanced care planning and chronic care management.
• Prescribe medications, including controlled substances per OH law.
• Order and review treatments and diagnostic tests.
• Communicate with nursing home staff regarding changes in condition.

While an interdisciplinary care team to provide medical care to patients in long term care facilities, you will have the opportunity to impact in a positive way the life and wellbeing of the elderly population.

We are currently seeking Nurse Practitioners provide medical services to patients living in long term care facilities.

NP ROLE AND RESPONSIBILITIES:

• Provide primary and preventative care for long term care patients residing in skilled nursing facilities.

• Manage Residents with acute change in condition.

• Advanced care planning and chronic care management.

• Prescribe appropriate medications, including controlled substances in accordance with OH Law.

• Order, review and/or follow-up with treatments and diagnostic testing.

• Communicate with the nursing home staff on a regular and as needed to discuss changes in condition.

• and/or the care plan.

• Collaborate with primary care physician.

NP QUALIFICATIONS AND EDUCATION REQUIREMENTS
