Job summary

CareMed Health Solutions in Ohio is seeking a Nurse Practitioner to provide primary and preventive care for residents in long-term care facilities within an interdisciplinary team. You will manage patients with acute changes, lead advance and chronic care planning, prescribe medications per Ohio law, order tests, and coordinate with PCPs and facility staff using an EMR.

Qualifications

• Current, unrestricted NP license in Ohio.
• Board Certification.
• DEA licensure.
• Clinical background in adult, family or geriatric specialties.
• Experience working in a long-term care setting.
• Ability to work with an EMR.
• Ability to manage patients with advanced illness and multiple chronic conditions.
• Ability to work within an interdisciplinary care team to coordinate patient care.

Responsibilities

• Provide primary and preventative care for long-term care patients residing in skilled nursing facilities.
• Manage residents with acute change in condition.
• Advanced care planning and chronic care management.
• Prescribe medications, including controlled substances in accordance with OH Law.
• Order, review and/or follow-up with treatments and diagnostic testing.
• Communicate with the nursing home staff on a regular and as needed to discuss changes in condition and the care plan.
• Collaborate with the primary care physician.
