CareMed Health Solutions in Nebraska City, NE seeks a licensed Nurse Practitioner to provide primary and preventive care for residents in long-term care facilities. You will manage residents with acute changes, coordinate with an interdisciplinary team, order tests, prescribe medications per Nebraska law, and support advanced care planning.

Qualified candidates hold Nebraska NP licensure, board certification, DEA registration, and experience in geriatrics or long-term care.

Qualifications

Responsibilities

We are currently seeking Nurse Practitioners provide medical services to patients living in long term care facilities.

NP ROLE AND RESPONSIBILITIES

• Provide primary and preventative care for long term care patients residing in skilled nursing facilities.
• Manage Residents with acute change in condition.
• Advanced care planning and chronic care management.
• Prescribe appropriate medications, including controlled substances in accordance with Nebraska Law.
• Order, review and/or follow-up with treatments and diagnostic testing.
• Communicate with the nursing home staff on a regular and as needed to discuss changes in condition.
• and/or the care plan.
• Collaborate with primary care physician.

NP Qualifications And Education Requirements

• Current, unrestricted NP license in the state of Nebraska.
• Board Certification.
• DEA licensure.
• Clinical background in adult, family or geriatric specialties.
• Experience working in a long-term care setting.
• Ability to work with an EMR.
• Ability to manage patients with advanced illness and multiple chronic conditions.
• Ability to work within an interdisciplinary care team to coordinate patient care.
• Psych experience a plus
