Qualifications

• 2-3 years Primary Care experience required.
• EPIC EMR is utilized system-wide.
• BLS certification required by the American Heart Association or American Red Cross.

Responsabilités

• Collaborate with physicians, nurses and care coordinators to support patient relationships.
• Provide preventive health visits, acute illness management and chronic disease care.
