Job summary

Qualifications

• Graduate of an accredited program of physical therapy.
• Current state Board of Physical Therapy licensure.
• BLS certification.
• One year of physical therapist experience (preferred).
• Three years recent clinical expertise in area of concentration (lead preferred).

Responsibilities

• Performs patient evaluations through history, systems review, and tests to obtain data.
• Establishes an individualized plan of care and selects treatment techniques addressing impairments.
• Conducts ongoing reassessment of patient and response to treatment.
• Initiates and manages the discharge plan beginning at the first visit.
• Supervises and delegates to ancillary staff and students per state law and department policy.
• Completes legible, accurate documentation within established timeframes.
• Communicates with patient, family, caregivers, and care team to optimize care using AIDET.
• Assesses educational needs of patient and provides education.
• Participates in quality improvement activities.
• Provides safe environment complying with safety standards.
• Performs within scope of practice as defined by state act.
