Job summary

Trinity Healthcare Talent Services is seeking a skilled Physical Therapist to join our 110-bed facility in New Hampshire, including a Mental Health Unit and Dementia Unit, where you will help patients regain mobility and function.

Qualified candidates hold a degree in Physical Therapy, current state licensure, and 1–2 years of relevant experience. This full-time role requires strong communication, teamwork, and documentation, with a Monday–Friday schedule and weekend rotation.

Qualifications

• Bachelor's, Master's, or Doctorate in Physical Therapy from an accredited program.
• Current state license to practice as a Physical Therapist.
• 1–2 years of experience in a similar setting preferred.

Responsibilities

• Conduct thorough evaluations of patients to assess physical abilities and limitations.
• Develop personalized treatment plans based on evaluation findings and patient goals.
• Implement treatments including therapeutic exercises, manual therapy, and modalities.
• Monitor and document patient progress and adjust plans as needed.
• Educate patients and families on exercises to improve function and prevent injuries.
• Collaborate with other healthcare professionals for coordinated care.
• Stay up-to-date with current PT research and techniques.
• Maintain accurate documentation of treatments and progress.
• Adhere to ethical, legal standards and facility policies.
