Physical Therapist (PT) – Home Health | Mission Home Health

Mission Home Health is seeking a skilled and compassionate Physical Therapist (PT) to join our team in the South Scottsdale and surrounding areas. This role offers the opportunity to make a meaningful impact by providing high-quality care to patients in their homes, helping them regain mobility and independence.

Position Details:

Compensation:

SOC: $115-$120

Revisits: $80-$90

Evals: $90-$100

Hourly Rate/NVA: $35-$40 (education and training)

Employment Type: Full-time, Monday-Friday, 8AM-5PM (we are flexible on scheduling!)

Service Area:South Scottsdale / Paradise Valley

Responsibilities:

1. Evaluate and assess patients' physical conditions and develop individualized treatment plans
2. Implement therapeutic exercises and techniques to improve mobility, strength, and coordination
3. Educate patients and caregivers on exercises, safety precautions, and self-care strategies
4. Collaborate with an interdisciplinary team to provide holistic, patient-centered care
5. Maintain accurate and timely documentation of patient progress and treatment plans

Requirements:

1. Active Physical Therapist license in the state of Arizona
2. Doctorate or Master’s degree in Physical Therapy
3. Minimum of one (1) year of experience as a Physical Therapist (home health experience preferred)
4. Current CPR certification
5. Reliable transportation with valid driver’s license and auto insurance
