Position Details

Location: Blue Pointe Healthcare Center, part of the CommuniCare Family of Companies. Our therapists are In-House Team Members, making a direct impact on residents' lives through therapy. We are a rapidly growing company with 140 centers across IN, MD, MO, OH, PA, VA, WV.

Qualifications

• Bachelor’s, Master’s, or Doctorate Degree in Physical Therapy from an accredited program
• Valid or pending State Physical Therapy License

Job Responsibilities

• Evaluate, review, and screen patients' medical charts and develop treatment plans based on physician orders and patient needs
• Implement treatment plans through direct therapy and educate team members, including supervising PTAs
• Participate in interdisciplinary meetings, relay clinical findings, and make recommendations; conduct staff and family training when appropriate
