Job Summary

The Physical Therapist shall screen, evaluate, re-evaluate, discharge plan, and revise established care plans under physician supervision.

Minimum Requirements

• Bachelor's, Master’s, or Doctorate in physical therapy from an accredited program
• Certification and licensure per state and national requirements
• Active state license
• Excellent organizational, time management, communication skills
• Knowledge of Functional Pathways policies, procedures, and clinical programs
• Ability to supervise Physical Therapy Assistants
• Completion of all required screenings, certifications, and licensure (varies by location)
