Key Responsibilities

• Patient Assessment and Evaluation:
• Conduct thorough initial evaluations and reassessments of patients in their home environment.
• Perform physical examinations to assess strength, range of motion, balance, and functional mobility.
• Develop individualized treatment plans based on patient needs and goals.
• Evaluate home safety and provide recommendations for modifications if necessary.
• Treatment Planning and Execution:
• Administer therapeutic interventions to improve mobility, reduce pain, and promote functional independence.
• Educate patients and caregivers on exercises, posture, safety, and techniques to maximize function and prevent injury.
• Use evidence-based treatment modalities such as manual therapy, therapeutic exercises, modalities (heat, cold, electrical stimulation), and functional training.
• Collaboration and Coordination:
• Work collaboratively with interdisciplinary team members, including physicians, nurses, and occupational therapists, to coordinate patient care.
• Communicate regularly with the clinical supervisor regarding patient progress, updates, and changes in care plans.
• Ensure timely documentation of patient visits, treatment progress, and outcomes.
• Patient Education and Home Program Development:
• Provide education on injury prevention, mobility strategies, and chronic condition management.
• Develop and instruct patients and families on home exercise programs (HEP) to promote self-management of their condition.
• Documentation and Reporting:
• Complete accurate and timely documentation of patient evaluations, treatment plans, progress notes, and outcomes in compliance with company policies and regulatory requirements.
• Ensure all documentation adheres to Medicare, Medicaid, and insurance provider standards.
• Quality Improvement:
• Participate in quality improvement initiatives and in-service training to stay up-to-date with industry standards and best practices.
• Provide feedback to the leadership team regarding opportunities for improving patient care and operational efficiencies.

Required Qualifications

• Education:
• Bachelor’s, Master’s, or Doctorate degree in Physical Therapy from an accredited program.
• Licensure/Certification:
• Active Physical Therapist license in the state of practice.
• CPR/BLS certification.
• Experience:
• Minimum of 1-2 years of clinical experience, with a preference for prior home health or outpatient rehabilitation experience.
• Skills/Competencies:
• Strong clinical reasoning and patient assessment skills.
• Ability to work independently with minimal supervision.
• Strong communication and interpersonal skills to collaborate with patients, families, and team members.
• Excellent time management and organizational skills.
• Knowledge of home health regulations, including Medicare and Medicaid.

Preferred Qualifications

• Experience in home health physical therapy.
• Familiarity with home health care documentation systems and electronic health records (EHR).
• Specialization or experience in areas such as geriatrics, orthopedics, or neurological rehabilitation.

Physical Requirements

• Ability to travel within the designated geographic area to visit patients in their homes.
• Physical ability to assist patients with mobility and perform therapeutic exercises.
• Occasional lifting and moving of equipment or patients, if necessary.

Working Conditions

• Full-time, with a flexible schedule to accommodate patient needs.
