Job summary

Concept Rehab & Engage Consulting in Charlevoix, MI seeks a full-time Physical Therapist to join our rehabilitation team. You will assess patients, develop PT plans, and deliver skilled interventions in a SNF setting with an emphasis on functional outcomes.

Qualifications

• Graduate of an accredited Physical Therapy program.
• SNF experience preferred but not required.

Responsibilities

• Evaluate patients and develop individualized physical therapy treatment plans.
• Provide skilled PT interventions.
• Improve strength, balance, mobility, and functional independence.
• Complete accurate clinical documentation.
• Communicate with nursing and interdisciplinary team members.
• Participate in care planning and discharge planning.
• Educate patients and caregivers regarding treatment and safety.
