Job summary

The Californian in Pasadena, CA is seeking a full-time Licensed Physical Therapist to join a supportive rehabilitation team at a skilled nursing facility across from Huntington Hospital. You will evaluate residents, create plans of care, and work with an interdisciplinary team to improve mobility and safety.

Qualifications

• Current California Physical Therapist license – required.
• CPR/BLS certification.
• Skilled nursing, post-acute, or long-term care experience preferred, but newer clinicians who are motivated to learn and grow are encouraged to apply.
• Strong communication and clinical decision-making skills.
• Ability to work effectively as part of an interdisciplinary team.
• Commitment to compassionate, resident-centered care.

Responsibilities

• Perform comprehensive physical therapy evaluations and develop individualized plans of care.
• Provide skilled PT interventions addressing mobility, transfers, gait, balance, strength, endurance, and fall prevention.
• Utilize standardized assessments to measure functional progress and outcomes.
• Establish meaningful, functional goals based on each resident's needs and prior level of function.
• Monitor progress and modify treatment plans based on the resident's clinical presentation and response to treatment.
• Provide patient and family education regarding mobility, safety, fall prevention, and home exercise programs.
• Collaborate with OT, SLP, nursing, physicians, and the interdisciplinary team.
• Participate in care planning and discharge planning to support safe transitions.
• Complete accurate and timely clinical documentation.
• Contribute to a positive, collaborative rehabilitation department focused on quality care and strong clinical outcomes.
